Hard moments shouldn't be faced alone. We stand beside our clients and their families, in the hospital, in mental health care and in everyday life, with the resources, referrals and structure they need to move forward.

In the hospital

We help you understand what's happening, ask the right questions, prepare for discharge and plan a safe return home, so your loved one's wishes are heard.

In mental health care

We help families find counselors, programs and support groups, stay connected with providers and make sure their loved one is treated with dignity.

In everyday life

From paperwork and insurance forms to tough family decisions, we help you sort through it, connect you with the right professionals and give you a clear plan.

Resources. Referrals. Structure. Someone in your corner.

Clinical treatment and legal advice come from licensed professionals.

Who It's For

What We Take Off Your Plate

We arrange appointments and rides, research and coordinate services, check in on care, raise concerns early, organize insurance and benefits paperwork, and share updates with authorized family members. We can also introduce elder law, advocacy and funding professionals.

Your Coordinator and Advocate

Your named coordinator is a single point of contact for doctors, caregivers, facilities and family. They can speak up for your loved one in care meetings and give you clear updates: on track, needs attention or urgent. They call within 24 hours of any important change.

When you need hands-on care, we share caregiver rates in writing first. Licensed medical professionals handle diagnosis, prescriptions and treatment.

How It Works

  1. Free consultation. Tell us what's going on. We'll let you know quickly whether and how we can help. Urgent situations get a same-day response.
  2. Care needs review. We meet your loved one, talk with family and current providers, and send a clear written summary of their needs within a few days.
  3. Recommendations. We share vetted options in writing, including costs. You stay in control of every decision.
  4. Coordination and advocacy. We schedule, coordinate providers and caregivers, attend care meetings, and speak up for your loved one.
  5. Ongoing oversight and updates. We check how things are going, share regular updates and call within 24 hours of any important change. If the plan isn't working, we change it.
