Information we collect
When you send a care inquiry, we collect your name, email address and any phone number or message you choose to provide. When you book a consultation, we collect the contact details you enter, your appointment selection and any message you include. If you contact us by phone, text or email, we receive the contact information and details you share with us.
Please keep online messages general. Leave out medical records, insurance identifiers, and other sensitive information about yourself or anyone else. The online inquiry and booking forms are intended for adults, including adults contacting us about care for someone else.
How we use and share information
We use the information you provide to respond to your request, discuss care options, and arrange or follow up on consultations. Wix hosts the site and provides the systems used to receive inquiries and manage appointments. Wix may process contact and appointment information to perform those functions. We may also disclose information when required by law.
